  • Review your medical insurance policy. It is your responsibility to call your carrier to discuss your coverage and any special bariatric surgery requirements. Our bariatric coordinator will submit your insurance information to your carrier to determine if your policy will cover the surgery – before any consults. Once we have heard from your insurance company, you will be notified. There is an option for self-pay if your insurance does not cover it.

    Most insurance companies require that any type of surgery for weight loss be pre-authorized. What this means is that if your insurance covers the surgery, additional documentation will need to be gathered along with completing all of the steps in this program required by Dr. Smith before the final pre-approval can be submitted to your insurance.

    Upon submission and approval of all required information in this form, Dr. Smith's bariatric coordinator will contact you to schedule an appointment.
